He doesn't need to dress up, he's scary enough. He throws on all dark clothing and dusts fake blood on his knuckles. You just put on some jeans, a greenish shirt, made some rips in them, rolled around in the dirt, did shit to your hand and gargled and threw up fake blood, grabbed a baseball bat and went as some psychotic f*cked up in the head mental ayslum patient.
