The Joker laughed as the pain has started killing him.
JOKER: Hahahahaha.... (*cough)... that actually is... (*cough).... pretty funny! Hahahahaha.....
The Joker kept laughing as he lied on the ground until he was running out of breath like his lungs were rupturing him. His final act was he gave a sinister grin on his face and his last laugh just before he dies.

Batman looked down at the Joker's corpse with a guilty conscience. He could've saved him no matter how bad he was.

Later outside at the Asylum, Gordon and his police squad arrived just in time for Batman's assistance. They even had an ambulance truck where Dracula, Martha, and Mavis sat waiting for
GORDON: All units, coordinate your... what the?
When the gates of Arkham Asylum opened, they saw Batman walking outside while carrying the Joker's corpse.

Gordon and the rest of his officers went speechless after witnessing the Joker's death. Robin was also there at the rooftops to help Batman but only to find he was carrying the Joker's body.
Dracula, Martha, and Mavis got out from the ambulance and gasped in shock at what Batman has been through outside.
DRACULA: Joker.... is dead?
MARTHA: What happened back in there?
Without saying a word, Batman gently placed the Joker's dead body on top of a police car and walked away like nothing happened.
GORDON: Batman, what happened?
Batman didn't even answer Gordon's question and just passed by some fellow officers. He even walked passed by Dracula, Martha, and Mavis like they were never there.
MAVIS: (*whisper) Are you okay?
Even Batman ignored Mavis as she was concerned about him. She could easily sense the guilty conscious inside him as Batman continued walking.
The Joker is dead... and the nightmare is all over. Everything will never be the same from now on.
