The boy was killed by Cato after Katniss set off the mines that were protecting the Career's food.
The girl died at the Cornucopia.
District Four:
The boy died at the Cornucopia.
The girl died in the tracker jacker incident that killed Glimmer.
District 5:
The boy died at the Cornucopia.
Foxface ate Peeta's poisonous berries.
District 6:
The boy died at the Cornucopia.
The girl died at the Cornucopia.

The boy died at the Cornucopia.
