You were now starting to get a bump, it was a big baby bump, since you were expecting twins. You told Cas about you going to see dean and he agreed, to let you take the twins to see Dean once they are born. Cas knew they weren't his children, but he was going to love them like they were his own flesh blood. You hated that Cas would give up everything to help you, same with Sam. Sam agreed to help with the twins since he was their uncle. John agreed to help as well, he wanted to make sure that his grandchildren were healthy and loved. You knew they were gonna have an amazing family, but except their father was in a mental institution and he had very bad anger problems and struggles from PTSD and depression.
You kept going to see Dean or deliver him letters, he refused to see you, Sam or John. He would also toss your letters and not care. He shut down, he built walls around himself refusing to let anybody else in. Dean was even refusing to eat or leave his room, he kept to himself, the nurses and doctors were force Dean to come out of his room, make him talk to the other patients. Dean would just glare at them and they would leave him alone.

The doctors would call John and update him on Dean's condition. Still the same, quiet, not eating, not sleeping or participating in activities with the other patients. Dean would just read books on lore, he would read about legends and he wondered if they did exist. He wondered if he could stop ghosts, vampires, werewolves, any monster.
Doctor: Dean, your dad wants to come visit you is that okay?
Dean just stays quiet, he didn't want to see John. Dean had so much anger in his heart, he couldn't forgive you, John or Sam.
Doctor: Your dad is bringing someone else too Dean.
Dean didn't want to see Sam. Dean didn't want to see any of his family.
A FEW HOURS LATER
You and John went to the hospital, the doctor took you both to Dean's room since he refused to leave it. Dean was laying on his bed, staring at the ceiling.

John: Hey son.
Dean just keeps staring at the ceiling, not bothering looking at you or John.
You: hey dean.
Dean still didn't say a thing.
John: Dean, you need to realize that we did this because we love you.
Dean then gets pissed and throws his books off his shelf, then he gets up and shoves John against the wall.
Dean: You thought putting me back in here would help?! I mean, what the hell kind of father are you?

John looked down, he felt guilty, he remembered leaving Dean in the hospital when he was kid and he can still hear Dean yelling for John to come back, but he didn't.
John: I'm sorry son...
Dean: I'm not your son! You're not my father.

Those words killed John, he tried to help his son, he wanted to be there for his sons and tell them that their mother was watching over them. John felt like he failed both Dean and Sam, but he failed his first son.
