Bellatrix Black was always a bit off her rocker.  It wasn't until her second miscarriage that she went mad with bloodlust.  She had always felt ashamed that she couldn't have children, insanely jealous of her sister, Narcissa and the child she bore.  She felt as though she was no longer worthy of being part of the noble and most ancient House of Black.  Eventually Bellatrix gave up hope of having children and devoted her time, energy, and soul to being a Death Eater.  After she had finished torturing Frank and Alice Longbottom, she turned on baby Neville.  The last bit of warmth in her heart convinced her to spare him, vowing to always have a watchful eye on him.  No one would have been able to tell, but something in her clicked that day, causing her to spare him.  Be it insanity or the last piece of decency left in her, no one will ever know.
