A girl gets reincarnated into Sirius' body. His soul is in there but was born with a defect and would have died, but pulled her soul in to help it. She spent 33 years as him while his soul grew. She mostly had control over his body, but later as his soul grew stronger under the pressure of Azkaban, she gradually gave control to him. He had full control by the time he saw Fudge and escaped. A few days after he saw Harry at Little Whinging, she took control of his body one last time to make her own body and lived as herself since.
