That's a sketch that John (most likely) made on the back of a letter from Louis de Manoel (most likely another of Francis' boyfriends, good friends with John during his European times) to John dated February 18, 1775.
The arm seems to be holding a caduceus/forked stick which would represent the Greek/Roman gods Hermes/Mercury.
