So I was doing research on Snape as normal and I come across this weird face. The 2 weirdest Snape Faces I saw were this one and the one where Snape killed Dumbledore. In the description of this photo it simply says "Snape during his time as one of Voldemort's death eaters." So I'm thinking this was after he got the dark mark. It must hurt really bad for his face to make that twisted expression if he's on anaesthetics.
