Step 1: Go to the bathroom which has a mirror. Face to the mirror. Close the door and turn off the lights.
Step 2: While keeping the lights off, chant "Slenderman" 13 times. Flick the light on and then off. You should see his vision behind you from the mirror. Don't be afraid, he is not real. It's just a vision. But if you will get scared, he will become real.
Step 3: Keep the lights off and open the door. Don't turn your back to the room. Make sure not to go in that bathroom for next 13 hours.
NOTE: You have to do this after midnight. The best hour to do this summoning is 00:00-03:00.
