How long has it been since they were together?: 2 years.
Why did they split up?: They were together before Tim became a proxy, so they separated when he disappeared. They didn't actually break up, she just felt abandoned.
Do they get back together?: Yes, after a bit of time.
What has changed?: Obviously, Tim has become a murderer. But also he's let himself go mentally and even physically. He's missed her a lot.
How do they find each other again?: Tim was in her town and was visiting a cafe, not realizing she worked there.
