Death

Jesse died from injuries obtained by falling from a great height. After destroying the Command Block, the Witherstorm let go of her and she fell into a pit inside the Wither Storm, later landing on the stone below missing the body of water nearby.

Minecraft: Story Mode/Characters

Jesse

Big Damn Hero: Jesse destroys the Command Block when the wither tentacle lets go of her making her fall into a pit inside the Witherstorm in the final battle, leading to her heroic sacrifice.

Heroic Sacrifice: Falls after destroying the Command Block when the wither tentacle lets go of her making her fall into a pit inside the Witherstorm, dying shortly after the fall. She survived long enough to see Reuben, Axel, Olivia, Petra, and Lukas one last time however.

Heroic Sacrifice: She destroyed the Command Block which destroyed and defeated the Witherstorm, saving everybody and the world, thus making her a hero, in the process, however, sacrificing herself.

Episode 4: A Block and a Hard Place

Jesse's death. It just feels terrible to have your main character player die during the journey. It can be very jarring for the person playing the game after the previous four episodes. By far the most heartbreaking reaction is Lukas begging Jesse not to die and that he needs her. Lukas simply comforts Jesse as she dies. Lukas's quiet "I'm here" is reminiscent of anyone who's ever done the same as someone they love passes away. 

Lukas:"Please, Jesse...I need you."

This interaction gets even more painful as earlier in the episode, after Jesse let Lukas go find the other Ocelots Lukas promises Jesse that he'll find his way back to her after her finds them. It's even more painful as Jesse was saddened and distraught to see him go. It's even more painful that earlier he and the others say that they don't want to lose her.

Flashback

Reuben:(Oinking to Jesse)(Translation)(I don't want to lose you, you understand?

Axel, Olivia, Petra, Lukas and I to Jesse:"I don't want to lose you, you understand?"

End of flashback.

Jesse's pained pitiful voice as she is dying is terrible to hear. She even tries to get up, probably to assure Lukas that she's alright.

Episode 5: Order Up!

Near the beginning of the episode, Lukas is seen framing Jesse's inventory that she left behind when she died. Petra will ask Lukas how he is doing. All in all, it is a pretty depressing moment to remember a good friend and secret crush is no longer there.
