When Jeff fist came to the mansion, much like Clockwork he had to have surgery to fix his smile. The entire time he was there, EJ (who was the resident doctor at the time) kept going on about how annoying and frankly quite stupid it is for people to let their wounds get so infected. Since then, Jeff has taken it upon himself to infect his still-not-completely-healed smile just to annoy Jack. The jokes on him though, as EJ once told him that he was on his own for treating it, and Jeff resorted to begging EJ to sterilize and clean his smile since Mr. Smiley Psycho didn't know how to do it himself
