Custom OC Name Form + Rules

Introduction:

Hello! This is a book where you PM me the completed form, and I send you 3-5 customized names for you to choose from (and if you don't like those, I'll send you another 3-5 names). Please note that you MUST tell me which name you picked, because I'll probably recommend/use the other names that you didn't choose.

Original Universe/Fandom (Specify the common names)

Fandoms with regular human names (But specify the preferred origin of the name, ex: Irish, Japanese, Korean, etc.)

(You may ask if I accept a fandom other than the ones listed).

Rules:

1. There is an unlimited number of times that you can request a custom name.

2. You must tell me which name you picked from the list of names I sent you; otherwise, I might request that same name to someone else accidentally.

3. I won't push it, but please PM me the form. Please don't comment the form.

4. Do not harass me, insult me, or judge the names I suggest. I accept constructive criticism, always, but know that I always give an original selection of names.

5. Please fill out the form the best to your ability. You do not have to fill all of it out if you lack some of the information (ex: if you can only fill one or two sections out then I will still accept it).

6. Use dashes or colons (Personality - trait, trait, and trait)
