Stories which fluctuate between tense/point of view for no apparent reason and without warning.
Sure, if your story is in present tense and you add a flashback in past tense, that makes sense. But if the time period doesn't change, neither should the tense.
Same with point of view. Suddenly changing from 3rd person omniscient to 1st person without warning or any kind of indication is really confusing and irritating.
It's stuff like this that makes me think I'm the only person who proofreads their stories.
