Ktulu pulled the pin of the grenade and threw it against where a bunch are and blew up almost instantly on impact. The Squad then killed the remaining mask, floor after floor became harder and harder, each floor has more mask than the last. It was on the 3rd floor when tragedy struck, will one of few that affects Rainbow.

Before the detonation of the Nitro, Lion scanned 36 tangos and eliminated 27 before Montange was hit by the nitro protecting his team, critically injured as being about 7 meters from the nitro. Shrapnel made the pain worse for him, as a dozen and a half had injected into his body.
Ash: Hold on Montange you'll make it, can't leave us now, stay with us
Montange: If it's my time, let it be, go now, I will not live longer now, save them , don't worry about me, save them, not me
Buck: C'mon you can't leave us, you have much more to have now
Montange took his last breaths as he passed away from a fatal explosion from the nitro being yo close, he didn't care he was about to die. His team was still alive and him risking it all was his main goal, not letting his team down.
