Chapter 1

Meredith: "What happened?" she asks running into the ER as Cristina and Richard try to stop her from looking into the room 
Cristina: "A bus before in front of him hit a telephone pole, he tried to swerve to miss it but-"
Meredith: "How bad is it?"
Richard: "Meredith-"
Cristina: "Meredith, you're gonna have to be strong." she says as Meredith pushes her out the way and looks into the room to see Bailey doing compressions on Derek as the machines continue to flatline
Meredith: "Derek! No. No." she sobs as they watch from outside the room as Bailey stops compressions and Mark and Sky look down at Derek with a sadness in their eyes as Bailey calls time of death "No! No! Derek! No! No! Derek! Derek!" she cries out in pain and slams her hand against the window.

She suddenly gasps awake and looks around before getting up and running down the hall

Bailey: "Back up. You're sucking all my oxygen." she says as Derek leans over her shoulder as the two of them plus Erica, Cristina, and Mark stand around a computer

Derek: "I'm trying to see."

Bailey: "It's not up yet. There's nothing to see."

Erica: "It's supposed to be posted at 4:00."

Mark: "It's 4:15."

Cristina: "Hit the refresh button. You have to reload the page."

Derek: "Yeah."

Bailey: "I'm hitting the refresh button every ten seconds. Just wait." she says smacking the woman's hand away from the keyboard

Meredith: "Is it up yet?" she asks coming over to the group

Bailey: "No." she says in annoyance

Derek: "Hi." he smiles at Meredith

Meredith: "Hi."

Alex: "Is it up yet?"

Bailey: "No! Wait."

Mark: "I'm thinking we move up a slot. We pass Hopkins, move up to number two."

Derek: "We're not passing Hopkins. We're gonna stay at number two."

Erica: "Mayo could surge ahead we could be three."

Derek: "Three? Pessimist."

Erica: "Realist. The rankings of teachings hospitals change based on any number of things."

Mark: "Ah, three is top five. I can live with three."

Alex: "Just hit the refresh button."

Bailey: "Get-stay back. I'm doing this."

Izzie: "Hey, is it up yet?" she asks coming over to the group with Sky next to her

Bailey: "No."

Sky: "Well, here. Just hit the refresh-"

Bailey: "I am hitting the refresh..." she suddenly jumps in her seat as she smacks Sky's hand suprising the woman and everyone around them

Sky: "Ow." she mutters as Mark grabs her shoulders to pull her a little away from the woman

Bailey: "Okay, all of you, back up. Stop hovering, stop speculating. The rankings will go up when they go up. We have no control over when they're posted. When it is up, I will say, "It is up." Until then, back up, shut up and leave me alone so I can push the-"

Meredith: "It's up." she says as they all move in closer to look at the screen

Bailey: "Oh. Oh, number one, Hopkins. Number two, Mayo."

Erica: "Told ya."

Bailey: "Three, Cleveland clinic?"

Derek: "What?"

Bailey: "Four, Mass Gen."

Mark: "What?"

Cristina: "Wait, wait, we moved down to five?"

Bailey: "No. We are not five." she says as they look down the list

Sky: "We're number 12."

Mark: "It's humiliating." he says as himself, Derek, and Sky walk along the halls towards a nurses desk

Derek: "Yeah."

Mark: "12? You're the best neurosurgeon in the country..."

Derek: "Thank you."

Mark: "In the world. And you are Sky Rivers. Every hospital bends over backwards for you."

Sky: "Thanks." she says as it's silent for a moment making Mark look over at the two with an expectant look

Mark: "Don't you want to say anything nice about me?"

Derek: "Uh, you're the best plastics guy at Seattle Grace."

Mark: "Nice, really nice." he says before turning to look at Sky waiting for her to say something

Sky: "I stopped insulting you out loud and in my mind. That's nice enough."

Derek: "Don't force me to praise you" he says as Rose comes over to put a chart away in between the two men "Hey, Rose."

Rose: "Hello Dr. Rivers. Dr. Sloan." she smiles at the two before walking away

Mark: "Mm." He hums at her

Sky: "Hi."

Derek: "She took it so well at the beginning. I was hoping we would be civilized. You know she's working my O.R."

Mark: "Civilized is out. It's gone. It's a trail of fairy dust. Now she's got delayed rage."

Derek: "Delayed rage?"

Sky: "Yeah, she was being big before, putting on a brave face. Now she's had time to think. Now she hates your guts."

Mark: "You have to break up with her."

Derek: "Well, that's the point. I did already."

Mark: "Well, you did it personally. Now you have to do it professionally. Good luck with that." he pats him on the shoulder before walking away

Derek: "Delayed rage..."

Sky: "I gotta go." she says as her pager beeps and he nods at her

Bailey: "Stop shivering. You're embarrassing me." she says as herself, Alex, Meredith, and Izzie stand outside in the ambulance bay

Izzie: "I didn't realize we were gonna be outside" she rubs her hands up and down her arms as Alex graba a blanket from a wheelchair to put over her shoulders "Oh, I'm ok. You don't have to do that."

Alex: "Bundle up. You're turning blue."

Meredtih: "Dr. Bailey, it is kind of cold out here."

Bailey: "Someone will come."

Alex: "No one's coming."

Izzie: "Maybe some old people with broken hips we can fix."

Bailey: "Okay, stop talking. We are Seattle Grace hospital, and rankings or no rankings, we are going to stand outside at the ready, on alert, and give the best care available to every single patient that arrives at our door. We may no longer be ranked the best, but I'm the best, and you're my residents, which means you're the best. It's a matter of pride, people."

Sky: "I was paged" she says coming outside and looks around the space "I don't see a trauma. Is there one on the way?"

Alex: "No."

Bailey: "Yes." they say at the same time as Sky gives them a confused look and Cristina comes out to them 

Cristina: "What are we doing?"

Bailey: "We're waiting."

Cristina: "For?"

Meredith: "A miracle."

Bailey: "Someone will come."

Cristina: "Dr. Bailey, you know that Mercy West is getting all the traumas first."

Bailey: "Someone will come."

Sky: "Ok, well while you guys wait I'm gonna go back into the warmth. At the very least we'll get to treat you guys for frostbite."

Meredith: "So we're standing out here in the hopes that someone is severly injured? Wishing and praying that someone is so hurt and so near death that the ambulance has to bring them here because we're closer than Mercy West?" Sky hears as the goes back through the doors

Bailey: "Yep."

Izzie: "Wow. You and God are cool with that?" she asks as Bailey narrows her eyes at her

Bailey: "Damn it. I forgot about God. All right. Let's just go on back inside and wait-" she sighs out as they move to go inside only to be stopped by the sound of tires sqeauling and a horn honking. They look to see a limo speeding into the ambulance bay before suddenly stopping itself as a woman in a prom dress steps out of it

Meredith: "Are you ok?"

Woman: "I drove a limousine on ice...all by myself, ten blocks."

Izzie: "Are you ok?" she asks helping another woman out of the passenger side

Woman 2: "My dress-there's blood all over my dress. What happened?"

Woman: "What happened was I got us to the hospital in one piece."

Izzie: "Ma'am, please try not to move, ok? Just sit here until we can get you a wheelchair. We need a wheelchair over here."

Woman: "We're not the patients. What's his name? Benny? Billy?"

Bailey: "Billy?"

Woman 2: "The limo driver."

Woman: "He flew through the windshield."

Woman 3: "Help him, please." she says when they open the backdoor to see a man laying across her lap as he gurgles blood

Bailey: "Uh, Yang, come around, help me. Get a board, uh, bring out a gurney, and a c-collar. And get Dr. Rivers out here."

Alex: "Dr. Rivers we need you outside." he says making her look up as they wheel a patient inside before rushing out and going over to the limo where Bailey and Cristina are in

Sky: "What happened?"

Bailey: "Flew through the windshield."

Sky: "Okay, get-uh,what-press it. Press." she says to Cristina and Bailey when blood starts to squirt out of the man 

Richard: "What do we have?" he asks when they wheel the man inside

Bailey: "Billy Matthews, 45 arterial bleed, haven't isolated the cause."

Richard: "Okay, I got this. Okay, let's get him in trauma one."

Bailey: "Okay, but I'm holding pressure over the bleed."

Richard: "I got this." he demands while pushing Bailey out the way

Bailey: "I just need-chie-"

Richard: "I got this Sky." he says when they enter the room and move to transfer the man onto a bed

Sky: "You sure? I can just be an extra set of hands."

Richard: "No I'm good. Go do work out there."

Sky: "Alright." she says after giving him a look before walking out the room

Callie: "Well, I gotta check in with Rivers, so-"

Erica: "Me, too."

Callie: "Rivers, anything else?" she asks when they move over to the desk where Sky and Bailey are at

Sky: "Uh, not right now." she says as they nod before going to walk away only to be stopped by Bailey

Bailey: "That's the problem. I'm charting. The chief took my one trauma. I gave up my clinic and now I'm charting" she says as Callie nods her head awkwardly before walking away and Bailey hands Erica a pair of scans "The chief wanted you to take a look at these. Billy Matthews. He's the limo driver. Multiple crush injuries from an M.V.C."

Erica: "Hemopericardium. Hemopericardium can lead to cardiac tamponade which can be detected by Beck's triad-increased J.V.P., hypotension and muffling of heart sounds" she says as Bailey gives her a confused look before to look arounnd for who should be talking to "This is a teaching hospital. I'm try to improve my teaching."

Bailey: "Well don't try it on me."

Mark: "Want to know how I teach?" he asks coming over to the desk

Erica: "No."

Sky: "You don't teach." she scoffs in amusement

Mark: "I'm like a guru. They come to me for help and I gently guide them along the path to truth and wisdom."

George: "Uh. Dr. Sloan, hi. Sarabeth Briar has got a facial lac, but she will only let you touch her."

Mark: "You paged me for a cut? What kind of moron are you, O'Malley? Are you a special moron from the isle of complete and utter morons?"

George: "I don't think so."

Mark: "Shut up" he demands before turning to wink at the other three doctors "Truth and wisdom."

Sky: "Like I said, he doesn't teach." she mutters while shaking her head before walking away

Lexie: "He's an ass."

Richard: "Am I to understand that you stole these traumas from Mercy West?" he asks when himself and Sky join Bailey and Cristina in the ambulance bay after finding the husbands to the women that came in earlier

Bailey: "I have no idea what you're talking about."

Richard: "Good job, cause now it's personal."

Sky: "Yang, come with me." she says as they run over to the ambulance

Paramedic: "We have a situation."

Sky: "Okay."

Paramedic: "58-year-old make, M.V.C., tachy and 160, B.P. 80 palp, decreased bilateral and breath sounds." he says as they watch them bring the man out on the gurney as a man blows air into a pen in the man's neck 

Sky: "What did you do?"

Paramedic: "I didn't do it. He did. G.I. Joe trached him at the field before I got there."

Sky: "With a pen? You trached this guy with an ink pen?"

Owen: "So?" he asks as she raises her eyebrows at him before shaking her head

Sky: "Move this guy inside now. Come on. Let's go. Move, move, move, move."

Cristina: "Pen...trach...hot."

Woman: "He has a pen in his throat! Michael! Oh God, Michael! Oh God, are you-is that a-is that a pen in his throat? Does he have a ballpoint pen in his throat?" she yells out after they move Michael into a private room

George: "You need to wait outside. Let the doctors work."

Woman: "But they have to be very careful with him. Michael's been under an incredible amout of stress lately and the last thing he needs now-"

Sky: "Grey, can you handle this woman, please?"

Meredith: "We're gonna take you to your husband. He's around the corner."

Woman: "Sweetheart, I'm here. Michael, I'm here, sweetheart."

Richard: "This is not her husband?" he asks when Meredith leads the woman out the room

George: "Nope."

Sarabeth: "Oh my God, Michael!" she calls out when she comes into the room

George: "It's her husband."

Sarabeth: "What happened to him?"

George: "Please come with me, ma'am. Please, this way."

Mark: "You trached a man in the field using a ballpoint pen?"

Owen: "That's all I had on me. What? It's not like I didn't clean it off with fresh snow."

Alex: "That's awesome."

Owen: "Really? You're going with a size 6 shiley tube on that?"

Mark: "Somebody get this guy away from me." he says annoyed as Sky chuckles under her breath a little

Sky: "Uh, would you mind waiting outside for a minute. We'll be right there." she asks as he nods his head before walking out

Bailey: "Uh, I'm gonna need a C.T. of, uh, this belly, cheif. Karev, get us a line."

Mark: "Looks like his trachea's crushed. I'm gonna need an O.R. and an extra set of hands. Who's available?"

Bailey: "Grey."

Mark: "The good Grey?"

Bailey: "The intern Grey."

Richard: "And I'll throw in O'Malley."

Mark: "Oh, nice. Perfect."

Cristina: "Oh sir, Dr. Hahn is working on multiple crash injuries. She wants you to take a look." she says coming over to the three when they come out Michael's room 

Richard: "I'm on my way."

Sky: "You're bleeding." she motions to the wound on his leg

Owen: "Oh, my car was right behind theirs at the intersection. I cut it on impact."

Richard: "And you triaged these men?"

Owen: "Major Owen Hunt, U.S. Army, second forward surgical. I'm a trauma surgeon. I just got home on leave."

Richard: "Major Hunt, Dr. Rivers. She's the head of our trauma."

Sky: "Nice to meet you."

Owen: "Likewise." he smiles as they shake hands

Richard: "Dr. Yang, take care of Major Hunt's leg."

Cristina: "Uh sir-I'm scrubbing in with you."

Richard: "After you take care of Major Hunt" he nods before grabbing Sky and leading her away before whispering in her ear "What do you think about him?"

Sky: "Major Hunt? I just met him, like 20 seconds ago. Why?"

Richard: "Just a thought I'm having."

Woman: "Excuse me. I was wondering if you could tell me how Michael is doing?" she asks when Sky walks past the room where she's sitting in

Sky: "Uh, he should be on the way to the O.R. pretty soon" she nods as the woman sighs out and Sky gives her a concerned look "Are you ok? You all have been through a lot tonight."

Woman: "It's not just that. It's Phil. And it's also Michael" she says as Sky sits down next to her "But I'm guessing you guys already knew that."

Sky: "No, I-I didn't know for sure, but-"

Woman: "It's pathetic, isn't it-a married woman cheats on her bestfriend with her husband. You can't go much lower than that."

Sky: "You'd be surprised how often that happens." she nods awkwardly to her

Woman: "I ran into Michael at the mall and we had lunch and a week later, another lunch and here we are...eight months later and Sarabeth has no idea. When you spend your life with someone and you have children together you think it will always be this amazing and this wonderful. You think that you'll always feel that kind of love, and-and-and I do, I...I do love Phil. i just..."

Sky: "Just what?"

Woman: "Well, little pieces of you get chipped away...by another person and...then you shave little pieces of yourself away so that you'll fit together and one day you look up and you don't even know who you are. I better get back to my husband, he'll wonder where I am."

Sky: "Okay" she nods as she watches the woman walk out the room and she gets up to see Meredith standing against the wall outside with a look on her face "You alright Grey?"

Meredith: "I'm good." she shakes out of her thoughts as Sky nods her head before walking away

Sarabeth: "Michael can live after they put a pen in the throat?" she asks as they stand in a room and look over Michael's xrays 

Bailey: "Oh, yes ma'am. The emergency trach helped him breath."

Sky: "Dr. Sloan needs to get him to the O.R. to sta-"

Sarabeth: "Surgery he needs surgery?"

Sky: "When your husband was pinned underneath the car he suffered a laceration to his windpipe, which Dr. Sloan needs to fix to stabilize his airway."

Bailey: "He also has some bruising around his kidney and a LAC on his liver."

Sarabeth: "Will he need surgery for that?"

Alex: "There's a chance the abdominal injuries will heal on their own, but if they get worse, yes, he will need surgery."

Sarabeth: "Two surgeries? I...I can't believe this is happening."

Sky: "Hold on. This-this is today?" she asks when seeing something on the file Alex handed to Bailey

Alex: "Tonight, midnight."

Sky: "Um, ma'am, there's also one other matter."

Sarabeth: "More? There's more?"

Sky: "Um, just according to billing, your insurance expires at midnight. So we're gonna treat you, of course, but just to make it easier on you later, is there another card or another insurance policy?"

Sarabeth: "My husband, he-he takes care of these things."

Bailey: "Uh, okay."

Richard: "Yang, Major Hunt all taken care of?" he asks when she joins them in the O.R.

Cristina: Uh, I did my best sir

Richard: Okay, come put some pressure on this bleeder for me. You know what I think? I think that number 12 ranking was a mistake. I think it was a clerical error, and I'm gonna start making calls about it as soon as I get out of this surgery.

Erica: So, I've been observing teaching styles. Shepherd likes to teach by thinking out loud, Bailey is blunt and direct and doesn't over-teach, Sloan...well, Sloan likes to berate and humiliate."

Richard: "I think if you really want to learn about teaching, you should talk to your students, find out how they learn best. Take Sky for example, she's a visual learner. She needs to be able to see what it is that she's doing. In order to do that she has to have had seen the procedure done before. Also found out that linking a cool procedure in with the teaching helps her pay attention more" he says as Erica thinks about his words "Yang?"

Erica: "Oh, that's not necessary, sir-"

Richard: "Are you serious about this or not, Dr. Hahn?"

Hahn: "Fine. Go." she says as Cristina looks at her with hesitance 

Cristina: "Uh, for me personally I learn better when I'm asked questions. The Socratic method!"

Hahn: "Well, that's the cornerstone of medical teaching. It's what we do in rounds."

Cristina: "Well not everyone does it. Some people...simply tell you what to do."

Hahn: "Who? Are you talking about me?" she asks defensively 

Richard: "It's ok Dr. Yang. Dr. Hahn is eager to learn. You can be honest." he says before the machines start to beep erractically

Erica: "He's crashing. Get me an aortic clamp."

Richard: "I can't get a handle on this bleeding in his liver. We may just have to start doing damage control soon. I see a tear in the renal artery. Yang, are you familiar with the single-layer continuous closure?"

Cristina: "Yes, sir. Of course."

Erica: "B.P.s still dropping."

Richard: "Okay, max out on levophed and keep pushing the blood. Get in there and stitch, Yang."

A little while later after finishing their surgeries, Derek and Mark come into the area where the three ladies are at and Izzie is having Betty sign some papers while Sky looks through a chart

Derek: "I was able to repair your husband's vertebrae and relieve pressure from his spine but, uh..."

Woman: "He...he's still paralyzed?"

Derek: "Yes I'm sorry."

Woman: "Oh, my God. Oh, Phil. And...and...and Michael. What about Michael?" she asks casting a glance at Sky who looks up at her from the chart in front of her 

Mark: "I repaired his trachea, but unfortunately his vocal chords were completely severed in the accident..."

Sarabeth: "He can't talk? He'll never be able to talk again?" she asks as Mark lowers his head in sadness

Woman: "We have to be strong-we have to be strong for Phil and for Michael."

Derek: "I'll be ready for Betty in O.R. one when you're done here." he says to Izzie who nods at him and Richard comes over as the other two male attendings walk away

Richard: "Um..."

Betty: "Is it Vincent? Tell me what's happened, please."

Richard: "Vincent came through the surgery. He's stable but still critical."

Betty: "Oh my God."

Richard: "This is Dr. Yang. She is going to be personally responsible for seeing to it that your husband makes it through the night."

Cristina: "I'm...I will keep you posted."

Betty: "Keep me posted...about what? Where am I? Where's Vincent?"

Izzie: "She has a subdural hematoma. Her memory resets every 30 seconds." she whispers to them

Cristina: "Perfect."

Derek: "Rose. Please look at me" he says when he comes over to where she's writing on the O.R. board "If you're gonna behave like this, you can't be in my O.R." he says making her sigh before put the marker down and turn to face him

Rose: "Okay, there's no good way to say this. But, I've been acting this way because I'm...I'm carrying your child, Derek" she says making his eyes widen before she laughs out "Ha! Gotcha."

Derek: "This is your idea of a joke?"

Rose: "Oh, please. You deserve it. Who do you think you are?" she asks before imitating him "If you're gonna keep behaving this way, you can't be in my O.R."

Derek: "You should transfer to a different speciality."

Rose: "You should transfer to a different speciality." she says back in her normal voice

Derek: "I'm a neurosurgeon."

Rose: "You shoulda thought of that while repeatedly assuring me that you were over your ex."

Derek: "Oh."

Rose: "I'll see you in the O.R. I'm not going anywhere." she calls out as she walks away from him and he turns to see Sky standing at the nurses desk

Derek: "Assuming you heard that?"

Sky: "Yep" she says as he hums while coming over to her and laying his head down on the desk "You sure she's not crazy? Kind of sounded like something a crazy person would say."

Derek: "I don't think she's crazy. At least I hope she's not."

Sky: "As much as I hate to say it, Mark's right. You gotta break up with her professionally. This won't end up well." she says as he hums again and she pats him on the back before walking away

Chapter 2

Meredith: "Put her on a monitor, order a portable chest. I'm gonna cut your top off."

Cristina: "It's  between my 9th and 10th intercostal spaces. There's no hemo or pneumothorax. My vitals are stable! Just pull it out."

Hunt: "I wouldn't."

Cristina: "Mind your own business!"

Meredith: "Who is this guy?"

Cristina: "Get this thing out of me. I already screwed up in front of the chief today once I can't...I messed up a suture and now he's looking at me like I'm number 12. Mer, please. No one gets to see me like this, okay?"

Richard: "Yang..."

Sky: "What the hell happened?!" she asks as they walk into the room and see Cristina laying on a bed with a piece of ice stuck in her stomach

Richard: "Get her on a monitor and order a portable chest."

Meredith: "I did."

Richard: "Did you check her respiratory function-"

Cristina: "I did. See? Breath sounds clear and equal."

Richard: "You be quiet."

Sky: "Grey, what would you advise as a course of treatment?"

Meredith: "I think we should leave it in until we get the chest x-ray and C.T. back."

Cristina: "I'm fine."

Meredith: "But what about infection? This thing is definitely melting dirty roof water into her body."

Cristina: "Which is why we should pull it out."

Owen: "Leave it right where it is. You get stabbed in the chest and you're lucky enough to still be breathing, you leave the knife in, at least until you figure out what's going on inside. Leave it in."

Cristina: "Take it out."

Richard: "Leave it in. In the meantime, since you know so much, you can teach your interns how to treat you. This is a good opportunity to get back to the basics."

Cristina: "But I need to check on Vincent Kenner."

Sky: "Grey, Vincent Kenner is your responsibility now."

Richard: "But bear in mind, he is my patient, and I'm not about to lose another trauma case today. Keep that man alive. Can I get a word with you, Dr. Hunt? Sky?" he says as the two follow him out the room and he talks to them about a procedure for one of Derek's patients

Derek: "What's going on?" he asks coming into the room where Richard, Sky, Bailey, Callie, and Owen in a patient's room

Richard: "We're going ahead with the freezing."

Derek: "Without my consent? No, I don't think so."

Owen: "You really want to be the doctor who doesn't give his all to keep a man from being wheelchair bound for the rest of his life?" he asks as Sky and Bailey look between the men awkwardly

Derek: "Get out of my patient's room!"

Owen: "I don't take orders from civilians."

Derek: "Listen you arrogant, little-"

Richard: "Shepherd." he stops him from moving closer to Owen

Derek: "Okay, right now this patient is stable. He may never walk again, but if you try this, he could throw a clot, go into organ failure, and he could die."

Richard: "We're going ahead with the freezing."

Derek: "Ever since this morning, you've been pissing on everything with a pulse trying to prove you're not number 12, and you're not. But right now, with this decision. You act like number 12. Good luck with the patient Dr. Torres." he storms out the room as they watch him

Callie: "That chill you'll feel is the cold saline rushing through your veins to lower your core body temperature."

Phil: "And if it works, I walk?"

Callie: "If it works, the hope is that you walk."

Phil: "Ooh. Wow. Okay, uh...I'm-I'm getting cold."

Callie: "His temp's still 96.8, chief. Shall we lay another cooling blanket on him?"

Richard: "That's up to you Dr. Torres. You're taking point. That's what the chief of a number one teaching hospital does. He handpicks talented surgeons and lets them take point. Right Dr. Bailey?"

Bailey: "Couldn't agree more, chief."

Phil: "Warm and toasty, that's what I'm feeling."

Sky: "His pulse is falling are you sure-"

Richard: "Yeah, down to 52."

Callie: "Yeah, it's normal for his pulse to fall."

Phil: "H-how much longer we got?" he asks shivering

Callie: "Not too much longer, okay? You're doing great."

Bailey: "Keep thinking warm and toasty."

Phil: "Okay. I'm trying."

Sky: "Good."

Bailey: "Number 12, my ass." she mutters making Richard chuckle

Alex: "Dr. Bailey." he calls her as she sighs before going to talk to him at the door 

Bailey: "Yeah?"

Alex: "Michael Briar's abdomen is more tender. Now I-I know I'm supposed to get a C.T., but if we start the surgery after midnight, he's not covered. We're basically bankrupting him. I say we skip the C.T."

Bailey: "Oh, you s-you say? No, you don't say. You ask. And the answer is, we don't skimp on medicine to save money. Get the C.T."

Callie: "Phillip, you're doing great. I'm giving you more sedative and a paralytic to stop your shivering, okay?"

Phil: "Okay, okay."

Callie: "Okay, we'll need to intubate, Sky."

Richard: "All right, coming down. One, two and three and down." they lay the bed flat before beginning to intubate him 

Callie: "He's down to 90 degress. I think that's, um, too low. I-is that too low?"

Richard: "You did all the research."

Callie: "I think it's too low" she says just as the machines start to beep erractically "Oh, my God."

Bailey: "Okay, so what now? Is this gonna resolve? Do they recommend lido or amiodarone? Dr. Torres?"

Sky: "Callie?" she asks softly when she sees Callie begin to become fluster

Bailey: "Uh, Dr. Torres?"

Richard: "He's barely got a pulse."

Callie: "Uh, uh, should we do CPR?"

Sky: "No, uh. Let's t-try to avoid jostling Shepherd's spinal decompression unless absolutely necessary."

Callie: "Oh, crap. Crap." she whispers out

Richard: "His heart is liekly to stop, soon. What are you gonna do, Dr. Torres?"

Callie: "I don't know" she whispers before speaking louder "I-I don't know. I don't know..."

Erica: "You started him on ECMO? You're taking out all his blood and replacing it?" she asks when she comes into the room 

Richard: "It was the only thing to do, Dr. Hahn. We don't want to jostle his spine, and this will give his heart some rest. Now we need you to monitor him until we get him back on. Now are you in or not?"

Erica: "All right, we all ready to wean?"

Richard: "Let's do it." 

Bailey: "It looks like he's tachycardic. Should we shock him, give him adenosine?"

Erica: "Well, normally I'd say yes, but this man's an ice cube."

Richard: "Dr. Torres, in any of your research, was there anything to suggest what to do in a situation like this?"

Callie: "I don't-I don't-I mean, um...I think I messed up here. I mean, I don't know. I mean, maybe-maybe-maybe he would've been fine on his own. Maybe-maybe-"

Erica: "All right, look at me. Look at me. Focus. You're trying something new. There's nothing wrong with new. Breaking new ground, pushing the boundaries, it comes with the job. So, Dr. Torres, take a breath, stop panicking and just tell us. What did your research say?"

Callie: "My research showed that...that, um...arrhythmias can occur at a certain threshold. His appear to be between...uh, 91.5 and 92 degrees."

Richard: "He's around 90 right now."

Callie: "Okay, so..."

Sky: "We could use the ECMO to help bring him back up."

Callie: "Yes, but not too quickly, because that can cause problems of its own."

Erica: "All right. Looks like we have some warming to do."

Derek: "My patient still alive?" he asks coming over to where Sky and Richard are standing outside Phil's room

Sky: "He's stable." she hands him the chart for him to look through and he moves his attention to Richard

Derek: "You let some soldier off the street talk you into something that could have killed a man."

Richard: "He deserved the chance."

Derek: "We have no way of knowing if it helped or hurt."

Richard: "He deserved the chance, Derek. He deserved the best, the most that this hospital could give him. I've let a lot of things slide over the past few years. I've been...soft."

Sky: "It's not about you being soft."

Richard: "Yeah, I think it's the fact that Meredith's here, the fact that Ellis' daughter is here." he says as Meredith comes over to them and Sky leans against the wall as Derek and her listen to the two

Meredith: "Chief. Vincent Kenner-we gave him 31 units of blood, fresh frozen plasma, multiple liters of L.R. We did everything we knew how to do. We just couldn't bring him back."

Richard: "You don't know how to do anything...none of you. You're halfway through your second year of residency, and you walk around this hospital like it's your God given right to be here. Well, it is not, Dr. Grey. I assure you. You are here because I allow it" he snaps making both Derek and Sky raise their eyebrows in shock at him "You are here to do what I say and the one thing I ask you to do-you can't even save-"

Derek: "Richard. We can't save everybody."

Richard: "We should try harder. We should try harder." he sighs before walking away as Sky and Derek give Meredith a soft look 

Lexie: "Well, you look pretty stable, which is good, because I can't let you go into your next surgery unless you're feeling strong and stable, okay?" she talks to Michael as Mark walks past the room and stops oiutside to listen to her "It happens. People make mistakes. You know, they sleep with the wrong person and they hide it. But if you ask me, it's the part that comes after that matters, the part where you make it right. And I think you're off to a good start." Mark listens to the words she says before looking down the hall and sees Sky talking to a nurse before walking off making a small smile come over his face

Rose: "I'm sorry about the hand." she comes over to where Derek is standing at a nurses desk

Derek: "These things happen."

Rose: "Not to me they don't. Or they didn't before you existed for me. Before you existed...I was good. Every doctor in this hospital wanted me in their OR. I was known as a damn good scrub nurse. And now I'm the crazy girl out of "Fatal Attraction" who goes around stabbing surgeons."

Derek: "Sorry. I'm so...I'm sorry."

Rose: "There's an opening in peds. I start on their service tomorrow. Again, very sorry about your hand." she walks away as Derek watches her with a sad look on his face and Sky comes over

Sky: "Are you okay?" she asks seeing the look on his face

Derek: "We broke it off. Personally and professionally."

Sky: "Hurt more than you expected it to?"

Derek: "Yeah it did." he sighs as she pats his shoulder a little in comfort and giving him a small smile

Callie: "Might have been the hypothermia treatment."

Derek: "Or my surgical skills."

Sky: "Oh, we'll never know!"

Meredith: "What's going on?" she asks coming over to where they're standing outside Phil's room

Callie: "Phillip can wiggle his toes."

Bailey: "Oh say it."

Derek: "Dont make me say it."

Bailey: "Please say it. Say it for me...even though it's technically not true yet."

Derek: "We made a man walk."

Bailey: "We made a man walk." she says with a chuckle as they others laugh a little 

Callie: "We made a man walk."

Bailey: "Oh." she sighs as herself, Sky, and Callie walk away leaving Derek and Meredith

Sky: "Hold the elevator." she calls out as she hurries to it

Mark: "Oh."

Sky: "Thanks." she breathes out when he stops the doors from shutting and she steps inside

Mark: "No problem" he nods and they stand in silence for a moment before he turns to face her "We're on the right track right?"

Sky: "What?"

Mark: "Being friends. Or somewhat friends...we're on the right track right?"

Sky: "I guess. Why?" she asks giving him a confused look

Mark: "Just...wanted to make sure. Seeing as how we're having to be around each other a lot of time."

Sky: "Okay..." she breathes out still giving him a confused look just as the doors open and they walk out "Uh, goodnight."

Mark: "Night." he watches as she walks away before a smile comes over his face and he goes another way

The next morning Richard has the entire staff meet in an OR

Richard Listen up, people. Listen up" he calls out as everyone continues to talk and he shuts the door hard to gain their attention "We used to be the best surgical program in the west, an upstart program that no one ever heard of that blew everyone out the water. Then overnight, we got old...entrenched and sloppy, just like everyone else. We've been resting on our laurels. You have all been failing-failing to take initiative, failing to ask the tough questions, failing to practice your skills, failing to give opportunity to practice skills. And I let it happen. So you have not failed. We have failed. Well, it stops, and it stops now. People sliding over general surgery as though they will never see anything but a heart or a brain through the course of their career, as though blood from the heart doesn't make its way through the whole body or the nerves don't travel past the brain stem. It's shoddy teaching, and it makes for shoddy surgeons. We've fialed. Get it? We all have. And it stops now. The surgical training protocol is officially changing, people. The bar has been raised. Consider yourself on notice." he finishes before walking out the room

Authors note: I don't know about anybody else but Richard and Owen annoyed me so much in these two episodes. Like Richard to me was kind of acting like a little kid throwing a tantrum and Owen was just acting like he owned the place and was doing too much for somebody who was technically new.
