Give credit to me somewhere in your book. Preferably the blurb or first chapter of your story.

RULES
- Fill out the form completely.
- Do NOT do in-line comments; put your whole request in one comment, or else I won't do the cover. It's annoying and overall tiring for me to keep up with your request this way.
- If you requested a cover, don't make the same request for another cover book. It's disrespectful to me and the other cover maker.
- Covers take time, what with school and all, so it'll take anywhere between a few days to a few weeks for your cover to be delivered.
- Your work must be in ENGLISH.
