Note: this is a Seo Moonjo dark romance fanfic idea
It takes places shortly after the series ends and the ending is a bit ambiguous (I hope there's eventually a season 2) so here's my imagination playing out for a bit:

Seo Moon-jo (the main antag) is very much alive and in complete control of Yoon Jong-woo (the main protag), so he convinces Jong-woo to move to the US with him and start a new cult. They learn English to be able to understand most americans around them, but they still prefer to speak Korean in public and private.
Moon-jo fell in love with Jong-woo at the first sight of him, well before he even knew of the villain's existance, so the fact that Moon-jo's actions and body language around his first love were constant smiling, staring, and recording him really creeped the younger man out. He'd start seeing him everywhere, and eventually even ran into him every time he went out in public. It made existing feel really weird and became so paranoid about him that he'd begun to be a creepy, violent outcast himself.
When Jong-woo inevitably found his girlfriend tied to a dentist's chair, he had a complete mental and psychotic break. Especially when Moon-jo gifted him a bracelet made of human teeth and specfically pointed out to him his most recent pull - her tooth. He had voiced that beautiful angry scream of his to Moon-jo's insane laughter, spitting in his face that he'll kill him. The cult leader couldn't hold back his giggly smile, blushing over feeling like his crush has just confessed his own love to him. All Moon-jo had known of love and affection in his life was violence fueled anger.
He couldn't hold back anymore, not after seeing his love be angry at the sight of his girlfriend passed out and bloody right next to him. Moon-jo forced a full on make-out session with Joon-woo, shoving his tongue in the scared younger man under him. That was it for Joon-woo. He fought and writhed strongly and fruitlessly against the killer above him, feeling violated and murderous, and seeing red all around him. Moon-jo refused to let up of his advances, squeezing Joon-woo's esophagus, feeling the heat rise until the weaker man finally gives up fighting, being close to passing out. He looks down at him, proud of his beautiful creation. He whispered deep into the ear of the exhausted man to kill them all.
In Joon-woo's mind, he was trapped in that room, binded to the chair where he was violently kissed against his will, believing that Moon-jo is the one murdering everyone else in the building. In reality, Moon-jo followed Joon-woo around as he obeyed the command of the villain with no hesitation, smiling his creepy smile with wide eyes and enlarged pupils. If it was possible, he would've had heart eyes taking in all the bloody murders committed before him by his one true love.
The events of the show's ending unfold, with Joon-woo testifying that he killed Moon-jo in self defense and that he sat helplessly while the other killed everybody. While he's still in the hospital, Moon-jo watches him sleep at night, and disappears by morning, making sure he's alright. Until one night, when the damaged man suddenly wakes from a ptsd dream of the war and apartment building horrors combining, he sees him there in the darkness. He can't believe his own two eyes, so he tries to shut his real quick and try to wake up for real, but this is real. Moon-jo is real and alive and in his very room. Before Joon-woo has a chance to realize what's happening and open his mouth to scream, he's there with his huge hand covering his mouth and his other hand gestures to be quiet. He smiles down at seeing his love so beautifully fucked up and bruised. He leaves the room without a word, greeting the night staff so kindly after being dressed like a doctor. Joon-woo can only stare at the slightly open door to his room.
Once Joon-woo is discharged, he tries his best to reclaim his life, but he fails. He can't hold down a job, he can't afford a place to live, he can't even get in contact with his girlfriend anymore, despite trying so every day. Moon-jo hates it. He hates how the traumatized man tries to go back to normal after all that. How could he just pretend to forget him and the cult he killed himself? Vowing to claim a lost puppy all to himself, Moon-jo captures Joon-woo's girlfriend again, just so he can trap him back into his arms. When the ptsd-ridden man receives a text from an unknown number, it's like time froze and all he could feel was the chill engolfing his spine.
