Lunatic is just used to mean
mentally ill, its use throughout history has remained relatively consistent only slight deviations till modern day. Though the word parts don't quite add up, at least to me anyways.  As Luna refers to the moon and the suffix "tic" refers to a process or state so the direct meaning of the two parts is moon process or moon state. This is likely best stated as, in my opinion, state caused/induced by the moon. This state is an ill state, most commonly of unrequited love. Most notably being Caligula at least that I can think of. So the full true meaning of the word "lunatic" as both directly made from the word and implications based on how it was used and things like it associated with the moon would be "state (of madness) induced by the moon".
