This is post endgame scenario where everybody survives(Yay!!). Avengers now live at the tower along with the Starks( Morgan is not there ,sorry!). Peter spends weekends at tower and sometimes many more days when May is working outside the city for post blip charity and resettlement(I don't know if it really a this!)

Tony's POV
How I didn't see it coming. How can I be so ignorant about him. Its all my fault that he is in there. He was just fine last night and I'm the morning or he wasn't. He always hides what he is going through ,all the mask he wears to pretend he is fine,just so he do not be pitied or become a burden on someone. Oh god how do I tell him ,he never is a burden ,he is my kid. A child can never be a burden over his parent. I will talk to him, just he wakes up ! God please !
Peter just wake up for me, please be ok.
Oh its all my fault.
A hand over Tony's shoulder brought him outside his mind where they knew he was choking himself over the blame of what Peter is going through. To be honest nobody knew what was happening inside the medbay.
Bruce and Dr. Helen was inside with the unconscious teenager for about 3 hours now and no word of his recovery was presented to the worried avengers waiting outside the room.
Where Tony was on the verge of breakdown , Steve was trying his best to keep him away from going to the guilt trip, Clint was never so silent. Natasha showed her emotional side for first time whereas Sam and Bucky tried not to think of the worst ,Scott kept on looking to the photo of Cassie. He just like Clint could totally understand what Tony was going through ,every second felt like hours for him. Watching your child unresponsive, unconscious ,hooked up to a number of tubes and wires is the last thing a parent would ever want to witness.
Just then the elevator's sound rang around the silent medbay. Pepper came running into the room where everyone was waiting, one attached to the room where Peter was being examined. She went to Tony ,who needed her the most at the time. As soon as he saw her ,he wrapped his arm around her black formals and finally broke down.
"I should have known Pepper, there was something wrong with him. Ever since the war ended h..he..was just different."
"No one could have known Tony. He was as cheerful as he always was" she tried to reassure him.
"NO HE WASN'T, we did noticed but non of us thought it was as important. He was less cheerful than before, he tend to get tired more sooner ,though it was more than enough of any human to go that long ,but he isn't just some human ,he is Spiderman for god sake. We just thought he was over coming the change of being 5 years lost. No he wasn't."
No one know what to say. He was true. Everyone saw that the kid when in one minute was bouncing off the floors ,in the next was sitting down like a tired adult. Some time he rammble about his day at school and friends and movies, later he would sit in silence like total zone out from everything around.
This is the third time in last ten days that he passed out, First it was last Monday ,when Happy went to school to pick him up, and when Peter didn't came out ,he went inside to look for him. The office said that Peter was taken home midday because he fell unconscious during his P.E.. But as Peter told them ,it was just due to late night patrolling, and they all believed.
Next time was when May called Tony that Peter would not come for internship as he was sick and 'sleeping' right now. Another stupid excuse he believed.
And now at the training session. He was fighting like a pro. Dodging punches and arrows. His spider sense was at peak to alert him of any danger or harm. But a second later like he froze. Mid battle. Before his senses would react ,a shield just hit him in the shoulder. It wasn't a big ,dangerous shot he hadn't taken before but he fell down, like the impact took the soul out of him. It took seconds for avengers to realise what happened. Tony ran towards the limp boy and tried to wake him up. But of no use." Bruce ,go and get Helen . get the medbay ready" he ordered as he picked up the boy and ran towards the elevator.
And now they all where here,outside the medbay ,waiting for something ,anything that assure them that Peter was fine. Just the boy being stupid enough to skip his breakfast or not telling about a last night patrolling injury ,he was just used to hiding that. But just wished for something ,that is not bad.
Just then Bruce came out of the room. His expression didn't gave any satisfactory emotions. He seemed worried like never before. Like he wanted to hide something but he has to tell ,face the consequence. Like he know everyone would hate the news ,hate the one who broke it to them. But he has know choice but to tell.
"Tony! H ..he..." Banner stammered while bringing all the comfort he can manage into his voice.
"Just speak out Brucie, no need to drag it long" Clint finally spoke.
Bruce opened his mouth to explain," to start with, I think after blip ,Peter's spider DNA got further enhanced due to unknown interaction with infinity stones bu....but his heart wasn't strong enough to handle that much of change. His enhanced healing kept on repairing the damage those enhanced DNA was creating when his heart finally gave up!!!!"( I do not know if it works like this ,but just bate with me)
Bruce could see the shock ran across the faces of everyone present, did they just lost the kid, the happy and optimistic glue that he had become in last few months. Is it so.
When finally Bruce snapped everyone out to reality.
"Tony. He need an arc reactor . I think its the only way!"
With a few seconds of pin drop silence ,he spoke to everyone's surprise.
"NO!"
"What do u mean by no ,Stark . the kid is dying. Why can't u agree to it." Steve said while not breaking the eye contact with the billionaire.
"Its not that easy!"
"What is not easy ,u lived with it. The reactor saved your life so why can't it saves Peter's. Its not a choice Tony ."Clint said while bucky and Sam nodded in agreement. They just gave a disgusted look of why he was being irrational at this crucial moment ,shouldn't he be Peter's mentor, hero ,the one the looked upon like a father figure.
But Natasha and Pepper didn't said anything ,they both could see what others were ignoring. The fear in the eyes of the philanthropist. He was scared , because he knows what it feels like ,like when u lose that peace/ piece of your heart.
" u don't understand" Tony mummbled
"What !!?"
"U DON'T UNDERSTAND, what it is like , to have that reactor ,
That discomfort ,that never ending pain , heat ,that mechanical sound that never stops , that blue light blinds you in all those sleepless nights. That feeling that you are no longer the same person , changed , missing out something ,something as important as a piece of your heart.
Feeling of not being a complete human.
The consequence are more psychological than physical. U never understand . non of you will ever understand. How do you think you will make that understand a teenager ,tell him now this is his life , what would you tell him Rogers,huh. ..go kid just live with it".
" we will help him, all of us will. I know its not what you want ,non of us want this ,but this isn't a choice ,we don't have a choice you also didn't ,and he would understand that. He is a smart kid ,and he knows the value of life and he will cherish it till his last breath" Steve answered in his calm and composed gesture ,he got what tony felt like,and he respect his feelings.
"I need to talk to his aunt ,she deserves to know. Bruce u prepare for the surgery.
Please take care of him. Promise me !" Tony said.
"Don't worry Tony ,he is a strong kid ,he will be fine" Bruce assure him .
Before any other word Tony left medbay.
" hello ! May parker.?"
"Mr.Stark, yes may this side . why are you being so formal "
" mm.. I just... Its about Peter.."
" what ,is he fine ... Tony ,...u still on the line"
" yes yes ,I am here ,it just I am sorry May , I know I promised to keep Peter safe till you return but today...it ..just happened ...and he fell unconscious..... And we took him to medbay.....and then Bruce said that all that blip caused him his heart ...and..and....its the only option ,the arc reactor.....
But ..I ...kn....know its not good but I can't do anything

He would be more than happy ,"
"WHAT?"
" His only wish was to be like you tony, he would be so much happy to have something so close to his mentor ,to be with him. And about he hating you , I assure you that can never happen. "
" You sure about it May, he would forgive me!"
"Yes Tony ,and what ever is the reason you are so scared for him to have the reactor , i am sure you would be by his side to help him with it. You will be there, right ?
Can you promise me that"
" I Promise you May , I will always be by his side.

His head was throbbing ,like someone hit him with a train.
The lights were not a problem cause they were already dim ,but it was hard for him to open his eyes. Every part of his body was aching ,but his chest ,it was different ,it felt different . it was aching more than anything ,he hear multiple equipment around him ,a steady beeping, drops coming down the IV, and a strange mechanical buzz ,though slow but it was coming from nearby.
"Hy underoos. So you finally decide to wake up!" It was his mentor. Slowing opening his eyes ,he realised he was in medbay. He tried to sit up but that excruciating pain in chest stopped him midway
"Hy ,slowdown Bambi, it would take time to heal "
Peter followed the eyes of his mentor which went from his eyes towards his chest .
An arc reactor was glowing on his bare chest. He kept looking at that thing that was literally coming out of his chest .
"I am so sorry kid , it was the only way to save you , as your heart gave up and......"
" its so COOL ,Ned would freak out when I'll tell him about it.
Hy Mr. Stark (while still looking at the reactor).
I look just like you"
The smile on the kid's face , released Tony from all his guilt. Now only he has to do is , stay by his side.
