Here's a summary of the form if you don't know:
This is supposed to be for a series I'm making, Called Linkle Stones! Gem Star Time, And this is for a school called Yumesaki Gakuen (Dreambloom Academy) And this is for people who want their OCS in the School as Students. Anyways, There are Four Classes (Yume, Hana, Himawari, and Momo.) And Nine Clubs. In each class, there are 30 students, which is 15 students, in both high school and middle school in 6 rows. in the clubs, however, there are 5 students in each club. (Including the leader.) since there are nine clubs, and 5 students in each club, that makes 40 students in the clubs in total. Now, there is a Student Council, they are known for working to make the school better.
