Peter was in a bottomless spiral. He would never be happy again. His best friend was missing in action, possibly dead, he had a body that wasn't his and Tony Stark was disappointed in him. Somehow that was worse than Ned being gone.
Ned had been friends with the insecure boy since they were nine years of age. Six years had passed since then yet nothing had changed between the two. Peter was beyond despondent at the news of his closest and only companion as any rational human would be.
When Peter was talking to Iron Man only weeks after Ned's vanishing he had made it out to be his own fault- as if he could have and should have stopped it from happening. He was the worst friend. Stark's words to his recount were soul-crushing to the fifteen year old boy. I expected more from you, kid.
As for his other "problem", he was transgender. A word which means someone who's gender identity does not fit the sex given to them at birth. Peter was born as a girl and he hated every second of his life because of it. The young male was on hormone blockers, waiting for his sixteenth birthday. The only reason Peter had to wait so long was because of the gender clinic that Peter attended. They did not want to make any irreversible changes to him because the specific doctor the boy saw had decided that it was just a phase and he would grow out of it. Peter and his Aunt May saw it in another light.
There was one more secret. One that meant Peter would never be able to live a normal teenage life.
He was Spider-Man.
