On the way back from a Leviathan hunt, Dean wasn't feeling very well. He had a high fever of 103 and had an unbearable pain in his stomach. Dean was in the backseat of the impala with a trash bag, in case he lost his lunch.
Castiel, sensing Dean's pain, appeared in the backseat next to him. "You are hurt?" Cas questioned. "I'll live," Dean replied sarcastically. Cas cocked his head to the right and stared at Dean concernedly. "I do hope so. I could not manage life without you. What is wrong anyway?" Dean smiled at Cas and said "I'm just a little bit sick. I have a stomachache and I just feel really bad." Cas, being an angel, did not understand sickness. However, he still tried his best to comfort Dean.
Dean lost all of his energy within the next few minutes and was no longer able to hold himself upright. His body fell sideways into Cas, who graciously accepted Dean's head on his shoulder. Cas wrapped his arm around Dean's shoulders, placing his hand on Dean's head. He rubbed Dean's head for a few minutes, until he replaced his hand with his head, tucking Dean's head into his neck.
Cas whispered to Sam, "he is out, much like the light." After a few minutes of thought, Cas asked Sam, "how do you tell a person that you love them?" Sam whipped around quickly, unaware that an angel could have romantic feelings. Cas, startled by Sam's sudden movement, jumped back. Dean, only halfway awake, repositioned himself, and Cas rubbed his head until he fell asleep again.
"Who do you love??" Sam asked Cas. Cas looked at the floorboards, and then to Dean. After staring at Dean for a few seconds, Cas looked back up at Sam for answers, and possible reassurance.
Sam was shocked, and very excited. He had wanted his brother and Cas to be together for a long time. "Y-you just tell him that- that you are in love with him, and that you couldn't imagine life without him." Cas nodded in thanks, and kissed Dean's forehead, before disappearing from the impala.
