I have to be honest, they either lack the staff to supervise all the kids, asked the ghosts to do it or simply don't give a shit.
Example 1: The Forbidden Forest
In the first book they have to go into the forbidden forest as punishment and Hagrid decides that they should just split up! Those are two kids with sticks, a lamp and a scared dog! I get that he might thought Draco was the greatest evil out there but seriously?! He could have given them at least something to defend themselves! They don't learn defensive spells to a level where they could actually defend themselves in the forest just yet! I doubt they even were ready in their 5th year! Not even mentioning that Hagrid hopefully had another weapon because if he ran out of arrows that would have been an issue.
Example 2: Literally any other place in Hogwarts.
If there are reports that kids turn up dead because they got lost and even the headmaster doesn't know all the rooms and hallways then you should do something about it! Even the marauders didn't get all the rooms and I have to admit that the room of wishes is a pretty big safety hazard. What if the students that went to hogwarts from 1939-1945 prepared in case something happened and stashed everything there?
Example 3: The troll
There weren't even any adults with the kids! wouldn't it be smarter to assume that the troll was there for something, gather all the kids and move them in safe groups with someone who could potentially fight a troll while the head student evacuated the rest to the dorms? And maybe do a quick headcount first. This is a boarding school. If not everyone is present there aren't that many places to check at.
Imagine someone yelled "We are missing Hermione!" and then Percy would have told McGonagall who would have fought tooth and nail with that troll for the one decent student she finally found.
I think she would have torn that troll appart. And if everyone looked after the kids, someone might consider looking after Quirrell. Because it is suspicious if the teacher for defenses against dark arts wouldn't even try to fight the troll. Which also should have been a red flag. Mind you, in the end it was taken down by three first years.
You would think he stormed in and yelled "Guys, some troll broke into the dungeons and I, as teacher of defense against the dark arts, took care of it so now everyone is safe and we just have to clean up."
I get that wasn't an option because he had a plan but still. They also could have kept all the kids in the hall. Supervised. Protected. With food.
And someone should have still said "Hermione is missing!" and watched McGonagall beat the world record Usain Bolt made finding her one decent student.
