When she was in the place where she was being experimented on her Multos took on its own shape in its dimension. Since it is connected to her it is a distorted version of what she looks like.
Even though only special cameras can let one see into the Multos dimension, Sam can see this shadow figure anyway because of her connection with it.
(Lol I know in her bio it says she doesn't put out Multos but hey it looked kinda cool as tears k)
