The night was thick with rain and the sharp tang of antiseptic.

Sirens tore through the darkness as the car hurtled down empty streets, tires slicing through puddles that reflected trembling streetlights. In the back seat, Chiara Fiore clutched her grandson to her chest, his small body burning with fever, shivering beneath her hands. Her fingers trembled as she pressed his heated forehead to her lips.

"Aurelius... stay with Nonna," she whispered, her voice fragile, almost breaking.

His breath came shallow, uneven, too quick, too faint.

Her phone slipped twice before she could steady it. Finally, she called.

"Come now," she said when Amelia answered. Her voice cracked under the weight of panic. "It's Aurelius."

By the time the car screeched to a halt, the hospital doors had already flung open. Nurses surged forward, lifting the boy from her arms as machines replaced her warmth.

Tests.
Blood.
A suffocating, endless waiting.

Then the doctor returned.

"Leukemia," he said.

The word floated between them, distant, almost unreal like it belonged to someone else's life. And yet, the next words shattered every illusion.

He needed a bone marrow transplant immediately.

Hours crawled forward in a silence heavy with fear, the hospital scouring every database, every possible donor.

There wasn't one.

Chiara's gaze drifted, unwillingly, across the room.

To the girl standing quietly near the wall.

Isabella.

Same eyes. Same delicate face. Fragile lungs, weak from asthma the twin who had always needed protection.

The twin who could save him.

A chill, sharp, and absolute, settled in Chiara's chest. Not grief. Not fear. Something darker.

Decision.

Before anyone could stop her, she gripped Isabella's shoulders and propelled her forward.

"Take her," she said.

The room froze.

"I don't care if she dies."

Her words echoed louder than the beeping monitors. Longer than the machines could measure.

The surgery began and stretched on for hours.

When the doors finally opened, the doctor's voice was measured, careful.

"The transplant was successful. Aurelius is stable."

Relief burst around her in trembling sobs.

And then the second sentence fell.

"The girl... did not wake."

Silence swallowed the room whole.

Relief curdled instantly into fear, the fear that Amelia would discover the truth.

Chiara paid the doctor to make the body disappear.

Money changed hands.

But conscience did not.

Instead of complying, the doctor chose mercy. He hid the child, nurtured her in secret, and when she opened her eyes a month later... he gave her a new name.

Her old life vanished.

When Amelia arrived, frantic and searching, Chiara gave her a lie.

"She ran off."

Years passed. Amelia searched.
Years passed. Isabella thrived.

And somewhere between loss and survival, fate waited.

Because the girl meant to die would one day hold the life of the woman who condemned her.

And when that day came

The truth would finally have a heartbeat.
