FORM + REQUEST HERE

Here is the chapter in which you will request (if you want? You can just creep through if you're bored- leave a vote/comment if you're feeling adventurous). Please copy & paste the form & fill in your answers- I will not accept a request that is not submitted on the form. These questions are to help me make a cover you're hopefully going to like. I don't do the whole "surprise me" type of thing.

As of November 9th, 2016: if you decide to use the cover I make you, you must use it for at least one week. Too many times I've made people covers only to see them replaced in a day or two & honestly, it feels like a waste of my time. If you know you are not going to use it for at least a week, please do not even bother requesting.

Now that I've covered everything (pun intended), let's move on to the reason you're here..
