2. Dedicate a chapter of your story to me.
3. Credit my shop and name on your story's description.
4. Use the cover for at least 2 weeks.

RULES

1. Do not steal or re-edit my work. If ever found guilty a legal action will be taken.
2. Finalize your form with clear answers. Do not submit it if you're still hesitating and still have things to change on your story or even on your profile.
3. The change of username or title will only be accepted before the process of your request.
4. Claim your request 1-2 days after I post it. Once failed it will be transferred into the premade book covers where other authors could benefit from it.
5. No redos, unless I made the mistake.
6. Please stop being demanding once and for all. I won't tolerate it even once.
7. Cancellation of a request will be accepted until a week after the day you requested.
8. Comment hey na na na below your form so I know you've read the rules. I won't accept the request without this and no second chances will be given.
9. Story link must be provided. Unless you're a VIP.
10. Reservation for the next batch will only be accepted once you claim your book cover. Please keep in mind that only 5 slots will be given.
