Your username
Description of the story
Category (teen fiction, fanfiction, etc.)
Whether or not it is published or about to be
(Condition)
Would you like me to advertise your story?
And a description or an idea of how you want it to look, like a suggestion for a background image
Also, if you want me to, when I put the book cover in the top image (that is how you will get it) I will 'advertise' it and tell people to go read it :) ....also, only if it's an actual thing yet...like I can't do it if you haven't published it yet

Just put it in the description of your story or something
Also, please be patient!!!! I might not get it up right away because I usually have a lot of school work to do or I might have forgotten about it. But I do give you permission to (not spam) message me the info again if it has been over 2 or 3 weeks of you requesting.
