Put less succinctly, in one of the many parallel worlds that mirror one another in the upsettingly complicated universe there was a different book by Nelan Barrow. The shifted events that fall like dominos create a significantly divergent plot with some remaining core elements. Due to this differing but mirroring circumstances, Cale Henituse is offered the opportunity to regress rather than transmigrate. He just wasn't anticipating having a new brother upon doing so.

Instead of transmigrating as Kim Rok Soo, Cale Henituse was sent back in time after he was reincarnated as Kim Rok Soo. But what he didn't expect, with the regression and soul merging, he was sent back after he was beaten up by Choi Han(which happened earlier than before), but the after effects were much worse than he imagined. aka: an OG! Cale harem (because he deserves all the love) with the fictional version of narcolepsy and sleep apnea.
