Premature ovarian failure also called the primary ovarian insufficiency is the loss of normal ovary functions before a woman reaches the age of 40. If the ovaries fail, they fail to produce normal quantity of hormone estrogen or release of eggs periodically. It majorly leads to infertility.
Premature ovarian failure may commonly be given the name premature menopause but both these conditions aren't alike. Women with premature ovarian failure can have rare periods for years and even have chances of getting pregnant while women with premature menopause don't have periods and cannot get pregnant.
Once the estrogen levels in your body is restored in this condition, you can prevent some problems like osteoporosis from arising.
Some of the premature ovarian failure symptoms include symptoms of menopause. They are mentioned below:
1. Vaginal dryness
2. Night sweats
3. Problem in conceiving
4. Hot flashes
5. Problem in concentrating
6. Lower sex desire
7. Irregular periods for years or may be gained after a pregnancy or after giving up birth control pills
When should you visit a doctor?
If you have skipped your menses for three months or more, then you should visit your doctor to know the cause. You may skip your periods for plethora of reasons such as stress, pregnancy, change in habit, dietary changes. It is best to get yourself evaluated when you find your menstrual cycle changing.
And, if you are not having any periods, then also you should speak to your doctor about it to know the reason.
In woman who have periodic ovarian function, the pituitary gland secretes hormones during the cycle which causes small eggs in the follicles in your ovaries to mature. Usually one or two follicles are filled with fluid every month which reach maturity. As soon as the follicles mature, they release an egg. The egg enters the fallopian tube where it gets fertilized by a sperm leading to pregnancy.
One of the causes of premature ovarian failures is loss of eggs. It may happen because of several reasons mentioned below:
1. Chromosomal problems: Some genetic disorders are linked with premature ovarian failure. It comprises of Turner's syndrome in which a woman has just one normal X chromosome and other fragile X chromosome.
2. Toxins: Radiations and chemotherapy may be one of the causes of premature ovarian failures. Toxins like smoking, virus, pesticide and chemical may also speed ovarian failure.
3. Immune condition response to ovarian tissue: Under this rare problem, the immunity system produces antibodies to fight your ovarian tissue thereby harming the follicles and eggs.
4. Unknown reasons: Sometimes there is no chromosomal defects, toxin issue, autoimmune problem, but still you may suffer from primary ovarian insufficiency.
Treatment for premature ovarian failure largely concentrates on the problem which occurs due to estrogen deficiency.
Estrogen therapy: The therapy can help you avoid problems like osteoporosis and cure hot flashes. Your doctor can recommend hormone progesterone if you have a uterus. Adding progesterone to your diet protects your uterus lining.
The blend of hormones can initiate your bleeding once again but it will not restore the ovarian functionality of your body. As per your health and preference, you should go for hormone therapy till the age of 50.
In elderly women, long term use of progestin therapy has been connected rise in risk of developing cardiovascular diseases and breast cancer.
Calcium and Vitamin D medications: They help in the prevention of osteoporosis. It is needed to give you sufficient supplementation needed for your bone density. For women from 19 to 50, 1000mg of calcium is important on daily basis which should be increased to 1200mg daily after you surpass 50.
However, the optimal dose of Vitamin D hasn't been suggested yet. But, one should consume around 800IU on a daily basis via food and supplements. If you have low Vitamin D, then you should consume higher dose.
Speaking about infertility
There isn't any treatment available to restore this health complication of premature ovarian failure. However, it is essential to understand and mourn for the loss of your ovary function and get counselling in this regard, if possible.
As pregnancy is not possible after this, some women along with their partner who wish to pursue pregnancy opt for IVF technology via donor eggs. The process includes removal of eggs from a donor and its fertilization in a lab with the partner's sperm. The fertilized egg is then sited in the uterus for implantation and development which will end up in making you a mother soon.
